What is UDID? (Unique Disability ID)
Unique Disability ID card (UDID card) is a single document of Identification for Persons with Disabilities. It is the universal ID that is accepted all across the country.
The UDID project is initiated by the Department of Empowerment of Persons with Disabilities, Ministry of Social Justice & Empowerment. It is an integrated system for issuance of Universal ID & Disability Certificates for Person with Disabilities (PwDs). Grassroot Academy facilitates the UDID process at the community level, ensuring that persons with disabilities (PwDs) can complete registration, assessment, and card issuance smoothly.

Data Collection and Pre-Screening
- Persons with disabilities are identified through community mapping and local networks.
- Required documents such as Aadhaar, address proof, medical reports, and photographs are
collected in advance.
- Pre-screening ensures that applicants meet eligibility requirements and are informed about
the medical assessment process.
Medical Assessment Facilitation
- Coordination with district hospitals and medical boards is carried out to arrange disability
assessments.
- Assistance is provided for scheduling appointments, arranging transportation, and
navigating hospital systems.
- Where necessary, support such as interpretation or mobility assistance is arranged to reduce
barriers during assessments.
Application Process
- Applications are filled on the official UDID portal (www.swavlambancard.gov.in).
- All documents are scanned, uploaded, and verified for accuracy.
- Applications are tracked online, and any corrections are made promptly to avoid rejection.
Follow-Up and Delivery
- Continuous follow-up is maintained with medical boards and government offices to ensure timely processing.
- Beneficiaries are updated on the progress of their applications.
- Once approved, UDID cards are delivered by post from department
Post-UDID Support
- After receiving the UDID card, persons with disabilities are guided to link their ID with
entitlements such as pensions, scholarships, concessions, and assistive devices.
-They are also connected to education, skill development, and livelihood opportunities.
- Beneficiaries are included in wider support programs for long-term empowerment.
